LHUNTSE DZONGKHAG ZHIWOG WELFARE SCHEME (LDZWS) -2008
Be it adopted by the Meeting among civil servants that in order to provide for the Welfare of the employees of the Dzongkhag and create resources for Semso.
1. Introduction
1.1 This scheme will be called Lhuntse Dzongkhag Zhiwog Welfare Scheme (LDZWS) – 2008.
1.2 The Lhuntse Dzongkhag Zhiwog Welfare Scheme, hereafter, referred to as Scheme.
1.3 It shall come into force with effect from 1st July 2008.
1.4 The Lhuntse Dzongkhag Zhiwog Welfare Scheme shall be legally binding on staffs of Dzongkhag Administration who are registered members.
2. Objective
2.1 The objectives of the Scheme are to:
I. provide financial assistance as cash grants to assist Members in times of emergencies and distress;
II. render physical services in times of need;
III. Promote relations among the staff & officials.
3. Membership
3.1 A permanent Dzongkhag Staff (except contract & part - time employees) are eligible to apply in application form- 1 and the declaration on dependent form-2 (LDZWS) for membership to the Scheme.
4. Capital
4.1 The Capital of the Scheme shall be composed of:-
I. Registration fee. II. Monthly members contribution: III. Returns on Investments; IV. Awards received in cash by Dzongkhag; and V. Receipt from donation and fund raising activities.
5. Registration Fee Nu. 300.00
5.1 An applicant shall pay a sum of Nu. 300.00 (Ngultrum Three Hundred) only as a registration fee for their mem- bership along with the duly filled application forms which shall be non refundable.
6. Monthly Contribution
6.1 A member shall contribute Nu. 150/- every month to the Scheme, which shall be deducted at source by the Administration & Finance Division.
6.2 In case of insufficient fund balance additional fund shall be contributed by each member.
6.3 The contribution collected shall be directly deposited in a Bank Account of the Scheme not later than 3rd working day following the date of disbursement.
6.4 A member who had availed Extra Ordinary Leave shall deposit their contribution as follows;
I. At least 50% in advance; and II. Balance upon joining from the Extra Ordinary Leave.
7. Termination of Membership
7.1 Membership will cease in the event of death, superannua- tion, transfer (outside Dzongkhag Administration), com- pulsory retirement, resignation and termination from the service.
7.2 The membership shall be terminated on the following grounds:
I. Convicted against Tsawa Sum as per National Security Act.
II. Attempted to weaken the financial credibility of the scheme.
III. Absconding/Missing without notification for a month or more.
8. Benefits
8.1 The amount of welfare grant shall be:
Nu. 15,000 in addition to 70% refund of his/her total contribution made in case of a members death shall be paid to his/her authorized primary nominee immediately by the LDZWS.
Nu. 15,000 on demise of a spouse
Nu. 10,000 on demise of parents/spouse’s parents or one direct dependant of a member (as per Form 2)
a) If a member does not have both the parents he/she can nominate one direct dependent, and
b) if either of his/her parent is living he/she cannot nominate direct dependent.
c) if a member does not have living spouse’s parents, nomination of direct dependent on behalf of spouse shall not be entertained.
v) Nu. 10,000 on demise of members’ children/one legally adopted child and one stepchild of one year and above (as per form 2).,br>
9. Physical assistance
9.1 In the case of death and illness of the member and the registered dependent, the office and the Scheme will pro- vide “Physical assistance” unless intimated as not neces- sary by the member.
9.2 The administration & Finance Division and Sector Head concerned shall determine in consultation with member the assistance required.
9.3 The staff/officials of Dzongkhag Administration and the members shall attend the funeral, if it is in Lhuntse and where practical send a representative outside Lhuntse.
10. Refund of Membership contribution
10.1 In the event of superannuation, transfer, compulsory retirement, resignation and termination of services or when s/he ceases his/her membership to the Scheme, 70% of his/her contribution shall be refunded, if s/he has not availed any benefits from the Scheme.
10.2 Members or nominee shall claim refund of membership contribution.
11. Management Committee
11.1 A Management Committee comprising of the Sector Heads shall be established. Dasho Dzongdag shall be the Chairman of the Management Committee.
11.2 The Chairman shall appoint an Executive Secretary and Treasurer from the member for a period of 1 year.
11.3 The Management Committee shall meet at the least twice a year and all decision taken by the committee shall be on consensus.
11.4 All members shall meet once in a year in June-July wherein scheme status shall be informed.
12. Roles and Responsibilities of the Management Committee
12.1 The Management Committee shall be responsible to en- sure that:- All plans, program transactions are as per the provisions of the Welfare scheme; Ledger/Registers are updated periodically; Accurate and true accounts are maintained on money received, payment and investment made; Statement of receipts and disbursements are prepared and published annually together with balance sheet;
12.2 The Management Committee shall approve and adopt the annual accounts and returns.
,b>13. Role and responsibilities of the Chairman
13.1 The Chairman shall:
I. Preside over all Management Committee meetings;
II. Appoint a member to officiate during the chairman’s absence; and
III. Conduct any other business relevant to LDZWS.
14. Roles and responsibilities of the Executive Secretary
14.1 The LDZWS Executive Secretary shall be responsible to ensure that:-
I. The contributions are deducted by AFD every month from LDZWS members.
II. The contributions are collected by the Treasurer and deposited in the Bank Account on or before 3rd working day following date of disbursement.
III. Proper and up to date accounts are maintained.
IV. Conduct the business of LDZWS,
V. Call the meetings of the Management Committee and record its proceedings.
VI. Maintain a current register of all the members, their dependants and beneficiaries with all documents to avoid future problems.
14.2 The Executive Secretary and Treasurer shall operate the Bank account jointly and perform all the duties assigned to him by the Management Committee;
15. Roles and responsibilities of the Treasurer
15.1 The Treasurer shall;
I. Be responsible for all the money
II. Make disbursement and maintain proper accounts periodically;
III. Prepare vouchers and pass for payment jointly with Executive Secretary;
IV. Maintain up–to-date accounts on receipt and payment; and
V. To assist Executive Secretary for management during misfortune; and
15.2 The Treasurer shall co-ordinate with AFD to deduct member’s monthly Contribution from the salary and these are deposited in the Bank.
16. Role and Responsibilities of Sectors Heads
16.1 The Sector Heads Shall;
I. Certify and forward all applications of membership to the Executive Secretary;
II. Certify and forward all the welfare claims and application to Executive Secretary; and
III. Inform the Executive Secretary of all transfers (out side Dzongkhag), resignation and death of a member.
17. Procedures,br>
17.1 All members shall complete membership registration Form- LDZWS /F-1 and Dependent Declaration Form- LDZWS/F-2 and submit to the Executive Secretary, LDZWS.
17.2 All the payment due to members shall be released by the Executive Secretary, Dzongkhag Staff Welfare Scheme in Bank Cheque/Cash toward payment to the beneficiaries.
17.3 Proper records such as Cash book, Ledgers, register etc. shall be maintained by the Treasurer to record all receipts and payments.
18. Annual Programme
18.1 All the members will be notified 2 weeks prior to the Annual General Meeting.
18.2 The Annual Accounts of the Zhiwog Welfare Scheme shall be audited annually by auditors appointed by the Management Committee.
18.3 The Audited Accounts together with Zhiwog Welfare Scheme Report shall be placed before the Annual General Meeting for adoption.
19. Amendment & Interpretation
19.1 The Management Committee shall be the sole authority to amend, revoke and repeal any articles of these rules.
19.2 The Management Committee shall be the sole authority responsible to interpret these rules in the event of dispute.
